'AI Assists, Humans Decide' Is a Design Constraint, Not a Slogan

The phrase is easy to say. Most of the engineering work is making it structurally true rather than procedurally true.
Procedurally true means a policy document says a human reviews AI output. Structurally true means the system will not let a clinical or programme decision execute without a named human accepting it — the constraint lives in the orchestrator, not the handbook.
The tell is what happens when the human is in a hurry. If review can be skipped under pressure, the constraint was procedural. If the workflow physically will not proceed without it, it was structural.
This matters most exactly when it is least convenient — under-resourced teams, high caseloads, urgent decisions. That is also when a slogan is most likely to quietly become optional.
