PACS lock-in is rarely a mistake made on purpose. It is what happens when nobody made the archive decision on purpose at all.
A PACS contract is usually signed as part of buying imaging hardware, which means the archive decision gets made by whoever is buying the scanner — not by whoever will need the images accessible in eight years.
A Vendor Neutral Archive decouples the two decisions. The scanner vendor can change; the twenty years of longitudinal imaging data does not have to move with it.
The cost of this discipline is a slightly harder procurement conversation today. The cost of skipping it is a forced, expensive migration the day the PACS vendor is no longer viable.
